Your First Visit
What to Bring
For your first visit, please be sure to arrive 15 minutes before your appointment time and bring the following information with you:
- Your completed Dental History and Patient Information forms which you can download using the buttons below.
- Your Dental insurance card (if you have Dental insurance).
- A list of current medications you are taking including dosage.
New Patient Forms
With our online patient forms, you have the option of speeding up your wait time at your next dental appointment at Bain Dental Group. Save yourself some time by completing the forms at home using a computer at home or work. Please call us if you need assistance; our team will be glad to help! Thank you!

Important Requests
A parent or guardian must accompany all patients under 18 years of age.
Please alert the office if you have a medical condition that may be of concern prior to treatment (i.e. diabetes, high blood pressure, artificial heart valves and joints, rheumatic fever, joint replacement, etc.).
Also please let us know if you are on any medication (i.e. heart medications, aspirin, anticoagulant therapy, etc.) OR require medication prior to dental cleanings (i.e antibiotics for pre-med.)

Comprehensive Dental Exam
Your visit includes a thorough examination of your teeth and gums, an oral cancer screening, and necessary low-radiation digital X-rays to assess your overall oral health.
Relax in Comfort
Settle into our reclining chairs and unwind with warm blankets and neck pillows. We’ve designed our environment to make your time with us as restful as possible.

In-house Dental Savings Plan
Bain Dental Group’s Dental Savings Plan makes quality care affordable—with low costs and none of the hassles of traditional insurance:

You may enroll your spouse and eligible dependents. Eligible dependents include your unmarried children to age 19, and full-time students to age 23.
Standard Plan
Who should purchase this plan? Anyone who is eligible to receive a standard dental cleaning.
Standard Plan Membership Includes:
- 2 Basic Cleanings per year
- 2 Basic Exams per year
- 1 Set of X-Rays per year (FMX/Pan)
- 2 Fluoride per year
- 1 Emergency Visit per year (ER visits include 1 Exam + 1 X-Ray)
- 20% off All Other Dental Care
(*Excludes Orthodontics, Oral Surgery & Products)
Pricing:
- New Adult Individual - $387
- New Child Individual 0-14yo - $349
- Renewal Adult - $365
Periodontal Plan
Who should purchase this plan? Anyone who is eligible to receive a deep dental cleaning.
Periodontal Plan Membership Includes:
- 4 Perio Maintenance visit per year
- 2 Basic Exams per year
- 1 Set of X-Rays per year (FMX/Pan)
- 2 Fluoride per year
- 1 Emergency Visit per year (ER visits include 1 Exam + 1 X-Ray)
- 20% off All Other Dental Care
(*Excludes Orthodontics, Oral Surgery & Products)
Pricing:
- New Individual - $1500
- Renewal Individual - $619
Family Plan
Who should purchase this plan? Any family member eligible for a deep dental cleaning.
Family Plan Membership Includes:
- 2 Basic Exams per year
